HealthChoice introduced its member and provider self-service portal, HealthChoice Connect, Jan.1, 2018, as the online source for claims and benefit information. With HealthChoice Connect, you are able to access up-to-date health care coverage information and resources any time, day or night. Visit www.healthchoiceconnect.com.
ClaimLink is available through Dec. 31, 2018, to view remittance advices, which are available for claims with dates of service prior to Jan. 1, 2018, and paid through June 29, 2018. For claims with dates of service in 2018, or 2017 and prior and paid after June 29, 2018, please visit the new provider portal at www.healthchoiceconnect.com.
Security is a primary concern of HealthChoice. You will need to register a unique user ID and password to gain access to your information.
IMPORTANT: You must first register as a provider for each location and network (HealthChoice, DOC, or DRS) for which you file claims. To register, network providers need to have the following information available:
- Your tax identification number associated with the personal identification number you are registering.
- Your ten-digit NPI number or your SSN (provider ID).
- Your PIN is located on each remittance advice. Or, you can contact our medical and dental claims administrator toll-free 800-323-4314.
After you complete your initial registration, you will be asked to create your own unique username and password.
Once you register as a provider, you can create delegates. For information on how to create a delegate, select the link Access ClaimLink above and navigate to the FAQs at the bottom of the homepage. Your delegate will need to enter the last four digits of their driver license number and the delegate code that is assigned after you register for ClaimLink.
If you forget your user ID or password, please utilize the Forgot User ID or Forgot Password links on the ClaimLink homepage.
