Filing a Claim for Health Benefits
HealthChoice Medicare Supplement Plans
Most providers file your claims with Medicare and then automatically file your claims with HealthChoice. To process your claim electronically, your and your dependents' Medicare numbers must be on file with the plan.
If you have to file your claim with HealthChoice yourself, once you receive your Medicare Summary Notice for Part A and Part B services, you can file your claim by sending a copy of the notice to customer care at:

HealthChoice will send you an Explanation of Benefits on all claims that are processed. Your EOB is also available through HealthChoice Connect. If you have not already registered, create a user name and password to access your information.
Claims Filing Deadline
Claims must be received by HealthChoice no later than 365 days following the date of service. For example, if the date of service is Feb. 1, 2019, the claim is accepted through Feb. 1, 2020.
